• Educational Status (Maximum Education) 
• Marital Status (Current Status)
• Khana Member (Male)

• Member of Khana (Hijra)
• Occupation (specify occupation if you are working)
• Annual Income (Mention the maximum amount you earn in 1 year) 
• Health information (confirm working, disabled, disabled)
• Other information (provide)
• Mention whether beneficiary (Government or Private) 
• Inform about residence (specify house owned, landless, rented house)

Applicants living destitute in old age or having no one to look after, landless, having 1 or 2 children below 16 years of age are eligible for widow's allowance. 

The income of the applicant should be a maximum of 12 thousand taka per year (i.e. in 1 year). 

If the applicant for widow's allowance is disabled or ill, he will be eligible for widow's allowance.
